Single-piece vs multi-piece dental implants?

The type of dental implant used in the procedure plays a crucial role in its long-term success.

While the conventional system utilizes multi-piece type of dental implants, Dr. Motiwala’s Smile in 3 Days technique involves the use of single-piece Swiss implants.

The multi-piece implants have several parts- the implant fixture, which is fixed into the bone, the abutment, which supports the crown/bridge, a screw which connects these two components. Sometimes the abutments alone can be of multiple units. These pieces are telescopically arranged into one another and fixed with screws. However, these screws tend to loosen with time and require periodic tightening. These periodic interventions require removal of the entire bridge/crown, correcting the screws and re- fixing them back. Additionally, the micro gaps between the multiple units facilitate seepage of food debris and bacterial growth leading to infection.

Single piece implants, on the other hand, are devoid of all these components and therefore do not require periodic evaluation. The final crowns/bridges are cemented onto them with no additional components involved.

Design considerations of single-piece implant vs multi-piece implant

The multi piece implant has a very rough, and uneven surface near the gumline which when exposed due to gum loss or bone resorption, is very difficult to clean. This will lead to more plaque accumulation causing the surrounding gum tissue to inflame and result in infection.

A single-piece implant, on the other hand has a highly polished and smooth metallic gold finished neck. This will aid in easy maintenance of oral hygiene even when the implant is exposed due to gum loss or bone resorption.

Number of implants

Full mouth dental restoration when performed using the conventional technique, involves placing 4, 6 or 8 implants in each arch. These number of implants are not adequate enough to support all the crowns. Although the aesthetics are taken care of, the new teeth do not function well at mastication, and might chip off in due course.

Conventional techniques of dental implant restorations require additional procedures. For the upper jaw restorations, the bone may be inadequate for implant placement as it can puncture the sinus. In these cases, the sinus lift procedure may be necessary for appropriate placement of the implant. This procedure is additional, and may attract surgical risks, additional costs, and delayed timelines.

Patients with completely missing teeth generally present with resorbed bones. the quality and quantity of the alveolar bone can be compromised. Additionally, the sinus in the upper jaw hampers effective implant placement. When using conventional implant system, it is very much necessary to enhance bone quantity as the implants are placed in this alveolar bone. It is often addressed by doing additional surgical procedures such as bone grafts, flap surgeries and/or sinus lifting procedures. However, these procedures will not only pose surgical risks, but also prolong the total duration of the treatment and cause additional costs.

technique sinus graft, bone grafting , flap surgeries is not required thus saving cost , reducing healing time and infection risks.

Likewise, placing the multi-piece implants used in conventional techniques requires flap surgeries where the gum tissue is cut open and the implants are fixed inside it. The flap surgeries require a healing time, involve a cost rise, and pose an infection risk.

When a patient undergoes a dental implant restoration with conventional approach, the process starts with extraction of teeth if needed, followed by a healing time. The parts of implants are then screwed in, and the measurements are taken for the bridge/crown. These measurements are sent to the commercial lab, for making the crown.
